Bayview Diving Connect Group

This form is for all prospective adult (18 years or older) participants in diving activities under the Bayview Outdoors umbrella.

If you are younger than 18 years of age, please have your parent or legal guardian complete the Bayview Outdoors Parental Consent form at https://bayviewguam.churchcenter.com/people/forms/461632.

You must check all applicable boxes to participate in our activities.

I will provide all required equipment and supplies for the activity at my own expense and will ensure said equipment is in proper working order. Neither Bayview Church nor the group facilitators will provide equipment or supplies to participants. *

I will adhere to the buddy system at all times while participating in the activities. *

I am in sufficient physical condition for the activity and will maintain at least that level of fitness while participating in Bayview Outdoors. *

I will conduct myself in a manner appropriate to a Christian church (see Bayview website for beliefs and core values). *

Certification *

I have the appropriate certification level, open water (PADI) or equivalent, to participate in group diving events. I will not do any dive where conditions or the dive plan exceed the limits of my certification or experience. I will present my dive certification card to the facilitator on demand.

Hold Harmless *

I will not hold Bayview Church and its agents (defined herein as group facilitators) liable to myself or to any other person whomever, for any injury to persons or damage to property pursuant to my participation in any Bayview Outdoors activity. I hereby agree to indemnify Bayview Church and its agents, and hold Bayview Church and its agents harmless, from any liability, loss, expense or claim (including, but not limited to reasonable attorneys' fees) arising out of such injury or damage. Further, I specifically agree to be responsible for, and indemnify and hold Bayview Church and its agents harmless from, any and all damages or expenses of whatever kind arising out of or caused by a burglary, theft, vandalism, malicious mischief or other illegal acts related to personal property I use to engage in or support a Bayview Outdoors activity.
